The Wisconsin Insurance Plan relies on the producer ("insurance agent" or licensed intermediary) to secure the needed information from the applicant and from other sources to properly complete the application. Scroll down for links to applications.

PRODUCERS, TAKE NOTE
If possible, apply 45 - 60 days before coverage is needed to begin. Sometimes coverage is delayed as we need time to underwrite. We will endeavor to be more accommodating for a new purchase.
Read the application. It will help you to complete it properly. Read this website and the notices we send you, too.
Review the information provided on the application with the applicant. Provide all the information requested. Don't skip questions or ignore information or items requested by us in italics (as in question 10).
We will not call you for information that is incomplete, missing, or is not clear. Usually, we will reject the application, return the deposit, and write to the customer and you requesting the information needed. No coverage will be in force. We communicate by mail.
Be sure the deposit is proper. Checks should be payable to WISCONSIN INSURANCE PLAN. One check per application. The amount of the minimum deposit varies by the limit of insurance requested for the Dwelling/Building. See page 2 of the application. If your customer has had repeated late payments to us on prior policies, we will require the full annual premium as a deposit. If you are in doubt about what deposit to submit, call us.
Be sure the application is readable. Repeated faxing of an application usually renders it unreadable. It will be rejected.
Sign the application. The producer signs, not the applicant. Print your name in the appropriate area.
Provide a short narrative to explain situations that are a bit different. Help us to understand the risk.
If we had previously declined to insure a property due to needed repairs and you are reapplying after the customer has made the requested repairs, indicate on the application that the repairs have been made.
Coverage will never begin earlier than the day after both a proper submission with deposit is received in the Plan's office and the risk is deemed acceptable by the Plan for coverage to begin. Coverage never is backdated. Coverage never begins until the Plan deems the risk acceptable. Coverage never begins without money.
We recommend submitting a deposit with the application even if the customer's premiums are escrowed by a lender. There is no guarantee that a timely payment will be received from a lender.
We are not required to insure every property submitted. We are not required to begin coverage as requested. We may decline to begin coverage until the inspection is completed, until we receive additional information, or for other reasons. We may start coverage differently than requested. Do not make promises concerning coverage to the applicant or lender.
You are not an agent of the Plan. You have no binding authority. Do not issue a binder or any coverage document. We will not honor it.

We prefer you use the complete on-line application. However, in the event the application does not function properly, the print and complete with pen applications are still available. See below.

If you have problems and the application does not print properly, do not submit it. Call us at (414) 291-5353 or e-mail us at thewip@execpc.com and we will get applications to you.
