Applications

The Wisconsin Insurance Plan relies on the producer ("insurance agent" or licensed intermediary) to secure the needed information from the applicant and from other sources to properly complete the application. Scroll down for links to applications.

PRODUCERS, TAKE NOTE

If possible, apply 45 - 60 days before coverage is needed to begin. Sometimes coverage is delayed as we need time to underwrite. We will endeavor to be more accommodating for a new purchase.

Read the application. It will help you to complete it properly. Read this website, too.

Review the information provided on the application with the applicant. Provide all information requested. Don't skip questions or ignore items requested by us in italics (as in question 10).

Be sure the application is readable. Repeated faxing of an application usually renders it unreadable.

Sign the application. The producer signs, not the applicant.

Provide a short narrative to explain situations that are a bit different. Help us to understand the risk.

We will not call you for information that is missing or is not clear. Usually, we will reject the application, return the deposit, and write to the customer and you requesting the information needed. No coverage will be in force.

We are not required to insure every property submitted. We are not required to begin coverage as requested.

We are not required to start coverage upon receipt of the submission. We underwrite. We may decline to begin coverage until we survey the premises, until we receive additional information, or for other reasons. If coverage is declined, the deposit is returned.

Coverage will never begin earlier than the day after both a proper submission with deposit is received in the Plan's office and the risk is deemed acceptable by the Plan for coverage to begin. Coverage is never backdated. Coverage never starts without money.

You are not an agent of the Plan. You have no binding authority. Do not issue a binder or any coverage document. We will not honor it.

APPLICATIONS - For Use By Producers Only -- Not for Customers.

We have two types of applications:

	Complete on-line and print applications.
	Print and complete with pen applications.

We prefer you use the complete on-line application. However, in the event the application does not function properly, the print and complete with pen applications are available. See below.

You must have Adobe Acrobat Reader. If you do not have this program, click on the Adobe icon below.

If you have problems and the application does not print properly, do not submit it. Call us at (414) 291-5353 or e-mail us at thewip@execpc.com and we will mail applications to you.

Applications for our Commercial Crime Program must be mailed to you. Call or e-mail us.

COMPLETE ON-LINE AND PRINT APPLICATIONS (Producers only).

	Click on the appropriate link below to access the application.
	To start, place your cursor on the Applicant line and click your mouse or press <tab>. You may have to click a few times.
	You can move through the application with the <tab> key and darken boxes by pressing <enter>.
	You can also move through the application with your mouse. You can darken a box by clicking the mouse. It is more efficient to use the mouse when answering questions that have boxes - like question 10.
	Please pay attention to the pop-up boxes which will assist you in completing the application.
	After you print the completed application, the producer must sign it. Mail it to the Plan with documentation of rejection and a deposit (optional). See Applying page of this website.
